Foundation Board and governance
The Toolkit is overseen by the Long Covid Toolkit Foundation.
Foundation Board members
Theo Hooghiemstra (chair). Theo has an administrative, legal and strategic background at the interface of data and law. He also has extensive managerial and supervisory experience, especially in healthcare and education. Theo knows several people with long covid. He hopes to use his administrative and legal experiences, skills and network to contribute to a better outlook for people with Long Covid through the Long Covid Toolkit Foundation.
Carla van der Weerdt (treasurer). Carla has a financial background and an extensive managerial and supervisory experience in various sectors. Because of long covid, she had to end her managerial role as Chief Risk Officer at Triodos Bank. By participating in the Long Covid Toolkit Foundation, she hopes to contribute to the well-being of all people with Long Covid.
Dirk de Jong (secretary). Dirk has a background as a legal advisor with a focus on ICT and privacy. He is pleased to support the goal of the Long Covid Toolkit Foundation in the interest of long covid patients. Dirk brings a broad experience in the healthcare sector in relation to the use and exchange of healthcare data from legal, strategic and supervisory roles.
Annette Bouws (general board member). Annette has a legal background with a focus on international contract law. For her MBA thesis she researched the impact of covid. Annette knows several people with long Covid, among which one of her best friends. Knowing how long Covid interferes with the daily life of patients she is committed to contribute to the goals of the Long Covid Toolkit Foundation.
Statutes
The Long Covid Toolkit Foundation is registered with the Dutch Chamber of Commerce under #93620306. We strive to be recognized as a Public Benefit Organization (in Dutch: Algemeen Nut Beogende Instelling (ANBI) by the Dutch Tax Office as soon as possible.

Governance
The Foundation Board oversees the operational team. The team and the Foudation Board are advised by the Patient Board, the Scientific / Clinical Board and the Advisory Board.
